What is a Fit-to-Work certificate?
If you are planning to return to work after a sickness period or starting a new job, your employer may request a fit-to-work certificate, also known as a fit note, to confirm that you are safe to return to work or carry out your new duties.

What is written in the certificate?
Our Fit-to-Work certificate will typically include the following:
- Your Details: Your name, date of birth and other basic details
- Fit-to-Work: Confirmation that there are no contraindications to you returning to your workplace or starting a new role
- Workplace Adjustments: A recommendation for any workplace adjustments that are needed to accommodate your needs (e.g. gradual return, reduced hours, avoiding heavy lifting etc)
- Sign and Stamp: The doctor will officially sign and stamp the document
- GMC-registered: Your certificate will clearly show the GMC number of the signing clinician, giving both you and your employer peace of mind that your case has been assessed by a competent professional.
What do I need to submit?
To provide you with a fit-to-work medical certificate, we need you to fill in our online medical questionnaire, which will request the following information:
- Your name, date of birth and basic details
- Contact information including an email address that you have access to
- Details about your job role and workplace
- Questions about your general physical health and medical history
Is this the same thing as an NHS MED3 Fit Note?
No – MED3 Fit Notes are only available through NHS services such as your own registered GP and are required to obtain UK government benefits. Merlin Health is a private service and the medical letter we provide you is not the same as an NHS MED3 Fit Note.
Which roles is this certificate unsuitable for?
Our fit-to-work medical certificate is only suitable for low-risk workplace roles. Roles that involve higher risk are unsuitable for an online assessment and require a face-to-face review with a healthcare professional. Some of the roles our certificates are unsuitable for include the following:
- Driving: Including HGV, Taxi, Bus, Ambulance and Motorsport Drivers
- Heavy Machinery: Including crane, forklift or other similar machinery
- Heights: Any roles involving working at heights
- Healthcare professionals, plant operators, motorway worker, mechanical handling, roles involving heavy lifting

What happens after I apply for my fit-to-work certificate
Once you have completed the medical questionnaire, the information will be reviewed by one of our doctors who will proceed to issue you a fit-to-work medical certificate if deemed appropriate, on the same day.
If after reviewing the information provided, the doctor feels that they are unable to issue you a certificate due to your medical background, we will issue you a full refund.
How long will it take to receive my fit-to-work certificate?
All requests submitted before 10pm (British Standard Time) have their certificates issued and sent through email the same day.
What information do I need to submit?
You will be prompted to fill in a medical questionnaire with details about yourself, your job role and your medical history. Please include accurate and clear information.
Who signs the certificate?
All our medical letters are signed by one of our GMC-registered doctors, all of whom are UK-based.
Can I get a refund?
If we are unable to issue you with a certificate due to your medical background or nature of work, we will issue you a full refund.
Once a doctor has assessed your case and your certificate has been issued, we are unable to provide a refund.
If the doctor feels I am ineligible for a certificate, will I be refunded?
Yes. If after reviewing the information submitted, the doctor thinks you are ineligible due to your medical background or nature of work, we will issue you a full refund.

If the doctor has any queries about your application, they may reach out to you through email or telephone.
