Antenatal or Maternity Notes

To issue you with a fit-to-cruise certificate, the doctor must be satisfied that your pregnancy is uncomplicated. To prove this, you must submit:

Maternity Notes: We need to see the latest notes your midwife or doctor have written during your recent antenatal appointments. They will usually include details about how your pregnancy is progressing at each appointment and whether there have been any complications. If you do not have these, please get in touch with your midwife to obtain them.

Without this information, we are unable to issue you a medical certificate.

*Please Note: Simply submitting the results of a scan, or brief details about your pregnancy are insufficient and do not confirm that your pregnancy is uncomplicated. If your antenatal care is taking place in the United Kingdom, you should have access to your maternity notes, which can be hand written or digital depending upon which part of the country your care is taking place in. If you do not have access, please get in touch with your midwife to arrange access to these notes.

(4) You agree to absolve both Merlin Health and our doctors of any liability for any adverse events affecting you or any other party at any point in time.

(5) You are aware that at Merlin Health, we do not offer a consultation service and our reports simply confirm that you have not reported any contraindications to travelling and nor does the evidence you have supplied us reveal any such contraindication to the best of our knowledge. This is not a form of medical consultation, diagnosis or treatment. No liability is accepted for any adverse events affecting you or another party at any point in time.

(6) You are aware that our certificates are intended to confirm you have no contraindication to travelling based on the available medical evidence. They are not a guarantee against adverse events. They are not an insurance policy. They are not valid as medical indemnity.

(7) You are aware that Merlin Health is not a replacement for a doctor's visit, nor is Merlin Health your primary doctor or GP, and healthcare professionals at Merlin Health are unable to access your NHS or regular GP medical records.

(8) You agree that no other healthcare professional has cautioned you against taking this trip.

(9) You have uploaded relevant medical notes you have available and have not purposefully withheld any information.

(10) You are aware that Merlin Health is unable to process refunds once our doctor has reviewed your request and you've been sent a letter written by them.

(11) You are aware that if a third party does not accept or rejects a Merlin Health letter or certificate, for any reason, neither Merlin Health nor our Doctors are responsible for any costs incurred.

(10) You are currently receiving antenatal care in the United Kingdom with a registered provider.

(11) You are aware that Merlin Health is unable to process refunds once our doctor has reviewed your request and you've been sent a letter written by them.

(12) You are aware that if a third party does not accept or rejects a Merlin Health letter or certificate, for any reason, neither Merlin Health nor our Doctors are responsible for any costs incurred.
